The MD Handoff tab in EPIC allows for clinicians to quickly access the vital details of a patient’s history and their background, as well as well tasks to accomplish. BUT it is only as useful as the information put into it, and updating the Handoff is key to providing the most useful information for other providers.
Instructions and template for the updated standardized MD Handoff are below, and copies will also be posted in the unit. You can use the smart phrase .SICUHANDOFF in the MD Handoff box to remind yourself of the expected elements. The biggest format change is the first box, which will include (1) a one-liner (not the full HPI) including date of injury/illness and mechanism, (2) PMH, (3) injury list, and (4) other notable ICU problems.
