- Discharge Summary Notes – This is a more extensive summary of the patient’s history and physical, treatments, etc. Patients discharged within 48 hours of admission do not need a dictated discharge summary. They do require discharge paperwork, normally done by the surgical service, but may be done by the SICU team to facilitate transfer (use template “discharge summary”). If done by the SICU team consultation with the surgical service may be necessary for specific plans, discharge medications, activity orders, f/u plans and phone numbers for appointments.
- Patients discharged more than 48 hours after admission require the above and need a discharge summary dictated. The discharge summary preparation is the responsibility of the surgical service.
